CMS Hand in Hand: A Training Series for Nursing Homes – The Centers for Medicare and Medicaid Services (CMS) worked with dementia care experts to develop this training that focuses on person-centered care for residents living with dementia. Training modules and toolkits can be downloaded.
The Eden Alternative – An international, non-profit organization dedicated to creating quality of life for Elders and their care partners, Eden shares resources for providers, and sponsors educational offerings and events.
Goals of Care – The NJ Goals of Care Coalition of NJ provides tools and information for health care providers, individuals and their families to help facilitate conversations about medical decisions at the end of life.
Institute for Healthcare Improvement (IHI) – Using evidence-based approaches, IHI offers information and resources on person and family centered care.
Paraprofessional Healthcare Institute (PHI) – PHI recognizes the pivotal role of direct care workers staff and shares best practices for hiring, training, supervising and supporting direct care workers.
Pioneer Network – The Pioneer Network offers a wealth of resources including guidance on culture change implementation, recent news and research findings.
Planetree International – Planetree is focused on person and family centered and person directed care in both hospital and long-term care settings. Resources include their Long-Term Care Improvement Guide.
Society for the Advancement of Gerontological Environments (SAGE) – brings together aging services providers, design professionals, regulators, residents and others who seek to develop physical environments that support resident-centered care. SAGE offers educational forums and evaluates senior living environments.

Institute for Healthcare Improvement (IHI) – In response to COVID-19 has launched a “rapid response network for nursing homes” with 20 minute virtual huddles at noon on Tuesdays and Thursdays that address key problems caused or exacerbated by COVID-19.
